Please confirm or cancel your appointment at least 24 hours in advance.

Please be on time for your appointment.

New patients: If you are unsure of any of your medication, please bring along your old prescription or the medication in their original packaging if possible.

Please remember to request forms/prescriptions and or medical certificates during your consultation.

Please note no medical certificates will be issued if Dr Khan did not physically examine the patient during a consultation.

If you had any previous tests done in the past 3 months, please bring the results or reports with you to the appointment to prevent unnecessary repeat testing.

If you are admitted as part of your consultation, the consult fee automatically falls away.

The practice will not be responsible for hospital authorization

For any emergencies late in the day or out of business hours may please go to the ER where the patient may be admitted (where necessary).

For planned admissions please wait for a call from the rooms to confirm your booked bed before reporting to the hospital.

For planned admissions, please make sure to have a list of your current medication and/or prescription.

Dr Khan works in conjunction with all main medical aids.

Please confirm with your medical aid as to which tests and treatments are covered by the option you are on.

Chronic applications will be done for chronic medication by the practice when necessary.

We will update any necessary information with your Medical Aid (Chronic Medication) on your request.

Consultation fee’s will be deducted from your day-to-day benefits. If there are no benefits available, you will be held liable for the account.

Some medical Aids requires authorization numbers, referral letters or co-payments in order to cover the fee. It is your duty to make sure what your Medical Aid requires before the appointment.
